Frequently Asked Questions: Health Questionnaire 

1. Q: Do I need to submit all three pages of the health questionnaire even if I think some of the sections are not relevant to me? 

A: Yes, regardless of whether you are a Medical elective or a Dental Erasmus student you must submit ALL three pages of the form. 

2. Q: Can I send an immunisation print-out from my GP / Doctor instead of completing page three of the form? 

A: No, even if you are submitting supporting documentation page three of the form must be completed and signed by your usual doctor or Occupational Health Service. 

3. Measles and Rubella Blood tests Q: I do not have a Rubella or Measles antibody blood test result; do I need to have a blood test? 

A: A blood test is not required provided your doctor can confirm you have had two doses of the MMR vaccine 

4. Tuberculosis Q: I have a BCG scar, do I still require a chest x-ray, Mantoux test result or interferon gamma test result? 

A: If you have not been living in the UK for the last twelve months you will need to provide a satisfactory chest x-ray result, OR Mantoux test result OR interferon gamma test result, written in English and taken within the last 6 months. If you have been resident in the UK for the last 12 months this is not required.
